To qualify for home health services a patient must:

  • Be homebound-requiring assistance to leave home.
  • Need intermittent care or instructions.
  • Require the services of a skilled healthcare professional that will coordinate and provide medical treatments or special procedures prescribed by the patient's physician.
  • Must be under the care of a physician who is willing to provide the Agency with orders.

Home Health Care is a program designed to assist the patient who is convalescing from an illness or disability, surgery, or who has had an accident or trauma.

Examples of conditions that may be referred for home care services are:

  • Diabetes Mellitus
  • High Blood Pressure
  • Orthopedic Conditions
  • Arthritis
  • Decubitus Ulcers
  • Surgical Wound Care
  • Stroke
  • Neurological Impairment
  • Cancer
  • Ostomy Care
  • Burns
  • COPD
  • CHF

Approved services are covered 100% by Medicare and Medi-Cal. Patients may be covered under insurance policies, or may pay privately for home health services. Nova Skilled Home Health, Inc. handles all insurance coverage requirements and processes all claims.
